  First Time Registration
 
  • Original and Photocopy of the worker's Passport;
    (Note: Photocopy only the first page & entry chop)
  • A copy of Jabatan Imigresen's letter of approval or visa;
  • Medical report from country of origin;
  • Registration fees payable to FOMEMA Sdn. Bhd.
  • Completed Registration Form.
    Note: *(1) Selection of clinic based on the list of clinics displayed at Fomema's Branch Office
    *(2) To view on clinic availability, please click here.
   
  Renewal Registration
(WORKER HAS PREVIOUSLY REGISTERED FOR MEDICAL EXAMINATION)
  • Registration fees; and
  • Completed Registration Form.
    Note: *(1) Selection of clinic based on list of clinics displayed at Fomema's Branch Office
    *(2) To view on clinic availability, please click here.
  • N.B.: If there is a change of passport, please attach a copy of the new passport (First Page)
